To make your first appointment as efficient as possible, we recommend you bring a photo ID, your insurance card (if applicable), a list of any current medications and allergies, recent dental records or x-rays if you have them, and your completed online forms. If you haven't filled out your forms ahead of time, just arrive 10 minutes early to complete them in the office.
